We are committed to ensuring continuity of care and minimizing disruption for members, particularly children receiving services in school-based settings.

CareOregon’s HealthShare OHP (One of the CCOs in Multnomah / Washington / Clackamas Counties) has terminated our provider contract effective June 24th, 2026 without a stated cause in the notice provided to us. This additionally applies to Jackson Care Connect which is also owned by CareOregon. If you are an active client, or potential client, and live in these regions, you have the option to request a change to another CCO. In the Portland region, this could be Trillium. In the Medford region, this could be AllCare.

It is your right as an Oregon Health Plan (OHP) member to choose your Coordinated Care Organization (CCO). In some cases, this may allow you to continue receiving services with your current provider, depending on eligibility and network participation. Below are the many pathways to facilitate this process at your earliest convenience:

Be sure to include your choice to switch your CCO. For example it could be:

Portland: CareOregon to Trillium

Medford: JacksonCareConnect to AllCare

(or vice versa, your choice)

  • Call OHA Client Services at 1-800-273-0557

    • Open from 8am – 5pm, Monday – Friday (Mondays and Fridays are the busiest call days)

  • Website: ohp.oregon.gov

    • Scroll down to the bottom of the page

    • Click on “Online help form”

    • Fill out the form with requested CCO

    • Submit

    • Member will receive a response by email (if they have one available) or phone call.

    • It is helpful if members can include 4 points of verification to ensure the correct case is being pulled up

      • Examples of verification points:

        • Full Name

        • Date of Birth

        • Last 4 Numbers of SSN

        • Full Address

        • OHP Card ID Number

        • Case Number

        • Phone Number
