Bringing Mental Health Care Directly to Oregon’s Most Vulnerable Communities

At Stronger Oregon, we know that healing doesn’t begin in an office—it begins with safety, stability, and connection. That’s why we’ve developed our Outreach Therapy & Stabilization Program, delivering individual therapy, family therapy, case management, and consultation services directly to individuals experiencing homelessness across Oregon.

This program is built on a simple belief:

Mental health care must meet people where they are—physically, emotionally, and socially.

Who We Serve

We support individuals and families across Oregon who are:

  • Experiencing homelessness or housing instability

  • Enrolled in, or eligible for, the Oregon Health Plan (OHP) (CCO assignment does not matter)

  • Living with untreated or under-treated mental health conditions

  • Navigating complex systems without adequate support

  • At risk of crisis, hospitalization, or long-term system involvement

Many individuals we serve face overlapping challenges including trauma, substance use, chronic stress, and barriers to consistent care.

What We Provide

Integrated Behavioral Health + Stabilization Services

Our program blends clinical care with real-world support to create meaningful, lasting change.

Individual Therapy

  • Trauma-informed, evidence-based therapy delivered in community settings

  • Flexible engagement (shelters, outreach sites, telehealth when appropriate)

  • Focus on stabilization, coping skills, and long-term recovery

Family Therapy

  • Support for families experiencing homelessness or reunification challenges

  • Conflict resolution, communication, and systems navigation

  • Strengthening family units to promote long-term stability

Case Management

  • Housing navigation and coordination

  • Access to food, transportation, and essential resources

  • HRSN (Health-Related Social Needs) screening and referrals

  • Benefits assistance (OHP, SNAP, TANF, SSI/SSDI navigation)

  • Ongoing follow-up and care coordination

Consultation & Community Support

  • Collaboration with shelters, schools, and community-based organizations

  • Behavioral health consultation for frontline staff

  • System navigation support for partner agencies

  • Training and guidance on trauma-informed care

A True Outreach Model

This program is not clinic-based—it is community-embedded.

We meet individuals:

  • In shelters and transitional housing programs

  • On the streets and in encampments

  • In schools and community hubs

  • Through coordinated referral networks and outreach events

We don’t wait for engagement—we build it through trust, consistency, and presence.

Bridging the Gap to Care

Many individuals experiencing homelessness are eligible for services but remain disconnected due to systemic barriers.

Our team helps by:

  • Enrolling individuals in OHP or supporting renewals

  • Providing immediate access to therapy and case management

  • Offering culturally responsive, low-barrier care

  • Coordinating across fragmented systems to reduce drop-off

This ensures that care is not only available—but actually accessible.

Stronger Oregon is building a scalable model to deliver outreach-based behavioral health services across both urban and rural communities.

  • Strengthen coordination with CCOs and community providers
